  • Your test request form is the Selph branded piece of paper attached to your test kit. It has your unique kit ID on it. On this form you'll write your personal details and your sample date and time.

    Please enclose this request form with your sample return - it's this piece of paper that lets the laboratory know which clinic, for example Selph, is requesting the test for you.

  • Our request form reminds you to write your sample date on your sample label. However, not every test requires this, and some just ask you to write name, date of birth and sample type on the sample tube label. If you have one of these tests, just follow the instructions and don't worry about writing sample date on there as well.

    Please remember to write your sample date and time on your paper form, and include it when you activate your test online.
