Which Request Form should I use with each test kit?

Our forms and test kits are interchangeable, so you can use any form with any kit.

The only thing to remember: Once you activate a kit with a specific form and ID, you must use that same form when sending your sample.

Using a different form than the one you activated could cause delays in receiving your results.
